9. Detailed work address
10. Phone and fax numbers of item 9
11. Current e-mail address
12. The exact dates of your stay in Russia
13. A list of Russian cities you intend to visit (up to 6, Moscow included)
14. The Russian consulate where you are going to apply for visa
(it suffices to indicate the city)
15. Accompanying persons:
(the same passport requirement as for you if a visa is needed).
16. Do you need ACCOMMODATION and what are
your preferences?
(a single room in a hotel priced starting from $50,
a single room in the university hostel priced about $25,
there are also options of sharing a room etc.)
If you need a RUSSIAN VISA,
please, indicate this in the registration form
and send us the scanned copy of your passport
(including the page with photo)
before May 15, 2007 by e-mail.
